Below is a list of documents that new Presumptive Eligibility (PE) workers at PE Qualified Entities can use for training in order to become certified to apply for PE for their consumers.

Below are helpful charts that new and existing PE workers can use as aids to help them with certain steps of the pre-screening and application process for consumers.

Who Can Apply for Individuals Chart - A chart that shows which individuals can apply for KanCare on behalf of each demographic category.

MAGI Building Individual Budgeting Units Chart - The IBU (Individual Budgeting Unit) Chart is what allows PE workers to determine the household size for every individual who is applying for PE.

KC-7004 Countable Exempt Income Chart - The official KanCare chart that lists what income is countable in a Medicaid determination and what income is exempt.

Income Conversion Chart - A chart that illustrates the calculations that need to be used to convert income amounts for PE applications.

F-8 KS Medical Standard - A chart to determine what medical program the consumer is eligible for depending on their IBU’s income amount.

Immigration Status Chart - A list of which categories of individuals are eligible or not eligible for Medicaid depending on their immigration status.

PE Tool Work Arounds - A necessary document that details the procedure for detailing with certain situations or errors that can arise with the PE Tool to ensure that every application is completed accurately.

Forms

Below are important forms PE workers will use while helping consumers apply for medical coverage.

Pre-Screening Tool - A tool designed to make it easy to see if a consumer is eligible for PE.

Fax Cover Sheet - A document the PE worker can choose to use as their cover sheet when faxing documents into the KanCare Clearinghouse.

KC-6200 Facilitator Authorization Form - English, Spanish - A document that allows the consumer to designate the PE worker or PE facility as a facilitator, which allows the worker or facility to receive follow-up correspondence related to that application and aid the consumer in completing the application.

KC-4501 Editable Facility Birth Reporting form for Medicaid Deemed Newborn - A birth form for when a baby is born to someone on Medicaid – that baby needs to be reported to the KanCare Clearinghouse as a Medicaid Deemed Newborn using this form.

Flyers and Other Information

KC-7101 PE Consumer Flyer - English, Spanish - This is a flyer that can be given to consumers to give a quick explainer of what the PE program is, how it operates, and how it can potentially benefit the consumer.

2021 MCO Benefits - English, Spanish - This is a document showing the benefits each Managed Care Organization (MCO) has for 2021, allowing the consumer to easily compare the different plans of each one before they make a decision about what is best for them.
