Use the processes below to notify a managed care organization (MCO) of a KanCare enrollee's pregnancy based on which MCO the enrollee uses.

Sunflower Health Plan encourages providers to complete notification of pregnancy within the portal.

Fax the above form to 1-877-353-6913.

Follow these instructions to complete notification of pregnancy in the UnitedHealthcare Community Plan provider portal:

Visit UHCProvider.com and log in.
Select "Clinical and Pharmacy" and then "Care Conductor and Notification of Pregnancy".
Enter the required information and complete the OBRAF on the last page.
