Follow these instructions to complete notification of pregnancy in the UnitedHealthcare Community Plan provider portal:

1. Visit UHCProvider.com and log in.
2. Select "Clinical and Pharmacy" and then "Care Conductor and Notification of Pregnancy".
3. Enter the required information and complete the OBRAF on the last page.
